About the role

The role of our Diabetes Nurse Educator is to provide direct clinical care and intervention. The role aims to reduce unnecessary hospital admissions by providing concentrated diabetes management support for clients with diabetes in the community.

This Community Health position promotes client-centred care within a Multidisciplinary Team. Supporting clients to gain the knowledge, skills, motivation and confidence they need to adapt to life with diabetes that fits their lifestyle, beliefs and culture.

Your duties include:

  • Accountable for the day-to-day Diabetes Education service ensuring the provision of high-quality care
  • Working in a multidisciplinary team to facilitate health partnerships and referral pathways with internal and external services providers to promote optimal client health and wellbeing
  • Strive to foster an interdisciplinary culture by providing support and guidance for health professionals in the provision of diabetes self-management education.
  • Plan, implement and evaluate programs in response to community needs and in consultation with other staff

Our ideal candidate:

  • Post Graduate Diabetes Educators Course accredited by Australian Diabetes Association (ADEA)
  • Current Division 1 Registration with the Australian Health Professionals Registration Agency (AHPRA)
  • At least 2 years' experience as a Nurse Educator
  • ADEA Credentialed Diabetes Educator

All Healthcare workers in Victoria are required to have received three doses of COVID-19 vaccination. Proof will need to be sighted during the recruitment process.
